years have performed expert opinion or consultation on various cases with Commercial Insurance, Industrial Mfg., Corporate
Legal Attorney's - Professional Firms - Boards - Claimant's
1. Professional Labor Fees: (Please Call)
a) Incident / accident, environmental health / chemical / biological exposure case evaluation, case development, review of required discovery
materials, development of interrogatory questions, site visits, background research, review of client’s materials and report
preparation.
b) Personal depositions, preparation, cancelation terms.
c) Court appearance, preparation, cancelation of court appearance terms
2. Expenses:
a) $50.00/ hour for travel time from our office to the site or location of consultation when traveling
two (2) or more hours
b) $0.55 / mile for miles traveled
d) All tolls, parking fees, public transportation, car rental fees, hotels, meals, etc. shall be paid by
Client.
e) All air travel tickets are to be purchased by the client and delivered prior to any flight dates.
f) Photos, (digital) embedded w/ brief description within electronic report $5.50 ea,
W/long full description $15. ea. (note these fees are for photos that are part of a sequential
depiction and description of a case review that either are part of a report or separated from the
report).
g) Phone $7./day, Computer $30./day; Printer/Copying .14/sheet plain B&W, quality B&W .19,
.60/ sheet plain color, $1.05 /sheet shiny white Color, Spiral Binding $5. w/o cover.
3. Retainer Fee: (Large Cases)
A minimum retainer of $1400. is expected prior to the start of any portion of a review and or
evaluation. Cases that appear large in scope, protracted time, where we recommend Professional
Expertise of: C.I.H., F.P.E., P.E., M.E., E.E., C.I.A. will have the retainer, hourly fee and expenses
negotiated.
4. TERMS of Invoices:
a). Invoices, shall be paid upon receipt or as agreed formally for a particular case. Invoices with attached details, will be generated based on
a minimum of 4 hours worked, or on a basis as specific aspects of tasks, works are completed
and depending on the case.
b). Estimated invoices for depositions, court appearances with corresponding preparation time and
expenses are expected to be paid prior to the scheduled dates. If the estimated amount is
greater than time spent in preparation, testimony or expenses, an adjustment will be made. All
depositions delays or cancelations are billed as stated, unless a stipulated cancelation or delay
period is mutually agreed upon prior to commencement of each case.
5. Payment:
Invoices are to be paid upon receipt, or as specified under the section TERMS period from receipt of
the invoice.

· Incident and Accident Reports
· First Report of accident and/or injury
· Investigations/Inspection (insured, insurance company, police, OSHA, etc.)
· Applicable standards & regulations (OSHA, NFPA, FM, API, USFDA, USCG, USFDA, EPA, etc.)
· Regulatory citations and or compliance officer’s notes
NOTE:
We are aware and understand that within some cases; expert consulting, opinion, evaluation(s), review(s) statements, and reports required for litigation are paid by either the attorney or their client. Thus, we require knowing, and fully understanding this arrangement prior to working on each case. Also, with protracted cases, when any changes are made from the initial agreement, as to who is responsible for payment of expert opinion, consulting, etc., we shall be made aware of any and all changes.
